ReDiscover helps men, women, and children who have limited income, no insurance or who are under-insured.We started out in Lee's Summit, MO as a group of volunteers in the basement of a local church. We incorporated in 1969. Over the years, the community has known our agency and our unique programs by several different names, including Research Mental Health, North Star, and Common Ground. In 2003, we permanently changed our agency name to ReDiscover.Under general direction of the Department Manager or Team Leader, the Outreach Case Manager provides assertive engagement to people experiencing mental health or substance abuse symptoms. The outreach case manager facilitates each person's engagement in services by creative service provision, problem solving and relationship building. The outreach case manager networks with other community providers and provides an enhanced safety net for the person served. The outreach case manager works in nontraditional ways to meet the needs of a client. Job duties include but are not limited to the following:POSITION OVERVIEW:Initiates good problem solving skills and uses unconventional thinking to locate and engage clients into servicesInteracts with clients in a non-traditional way to meet them where they are to engage them into the services that best fit their individual needsResponsive to clients in various stages of psychiatric distress or substance use in order to engage and stabilize symptomsDemonstrates an ability to develop rapport with a variety of diverse clientele with diverse needsDisplays Proficient in trauma informed care, stages of change, and motivational interviewingBuilds and maintains an extensive knowledge of community resources and has the ability to match client with eligible resourcesDemonstrates the ability to think outside of the box to eliminate barriers to service, resources, and engagementDemonstrates an ability to assess needs based on understanding of the whole person and then matches those needs to resourcesQuickly acts on knowledge of community resources to address client needsSkilled in resolving crises and/or de-escalating clients, staff, community and/or stakeholdersMaintains and initiates clear, concise communication that is timely and accurate with all referral sources involved in the service provisionWorks collaboratively and respectfully with internal agency providers and works cooperatively to make the best decision for service provision for the clientWorks collaboratively and respectfully with external interdisciplinary agency providers to build healthy partnerships and quality careDevelops and maintains positive relationships with vendors/community partnersRepresents ReDiscover in a positive professional manner at all timesOther duties as assignedQualificationsMINIMUM QUALIFICATIONS:Bachelor's Degree in a social science related field. PREFERRED QUALIFICATIONS:Two (2) year of direct service to the target population or other related human service experience preferred.Experience in a mental health or substance abuse setting and/or with clients who have co-occurring mental health disorders is preferred.LICENSE/CERTIFICATION: The outreach case manager must complete a Case Management orientation.A valid automobile operator's license, appropriate automobile insurance and a safe, reliable, registered automobile available for use during work hours is required.The ability to work with all persons with multiple challenges (mental illness, substance abuse, homelessness, physical challenges, developmental disabilities, etc.).SKILLS: The Outreach Case Manager must possess a working knowledge and skill proficiency to perform clinical, outreach and case management services.The ability to work with all persons with multiple challenges (mental illness, substance abuse, physical challenges, developmental disabilities, etc.).SALARY AND BENEFITS:ReDiscover offers a competitive salary, affordable medical and dental plans, 403(b) savings plan, and tuition assistance, along with other benefits.
