As a Transplant Coordinator RN, you will play a crucial role in managing pre-and post-transplant processes, ensuring seamless coordination between patients, families, and multidisciplinary medical teams. This position requires a blend of clinical expertise in transplant services, exceptional communication skills, and a deep commitment to patient care. Coordinate all aspects of care for transplant patients. Educate patients and families about the transplant process and post-care requirements. Manage patient assessment, pre-transplant evaluations, and post-transplant follow-up. Collaborate with physicians, surgeons, and other healthcare professionals to ensure comprehensive care. Maintain accurate and detailed patient records and transplant documentation. Participate in clinical research and promote continuous improvement in transplant services. Strong clinical skills with a focus on transplant medicine. Excellent communication and interpersonal abilities. Detail-oriented with strong organizational skills. Ability to handle emotional situations with professionalism. Capability to work autonomously and in a team. Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Current RN license in the state of practice. Certification in Clinical Transplant Coordination (CCTC) preferred.
