Hospice Admissions Registered Nurse (RN)-$5,000.00 Sign-on Bonus

The Admissions Nurse is responsible for educating, assessing and
admission of potential patients/families. Upon patient/family
admission, the Admissions Nurse develops a Plan of Care and obtains
other necessary admission documentation. If your heart draws you to

Possess the Following Requirements: Associate's or Bachelor's
degree in Nursing Current unencumbered RN license in Kansas and
Missouri (or Missouri multi-state license) Minimum of two years
relevant RN or Admissions RN experience Reliable transportation and
current auto insurance CPR certification Prior hospice and/or

information and assessing immediate or emergent needs of
patients/families; arranges appropriate care at the time of
admission. Assessing potential patients/families for admission into
Hospice, Palliative Home Care, or Hospice Houses programs
Establishing initial plan of care, obtaining medication and DME
orders, explaining and obtaining consent forms, as needed for each
program Documenting patient assessment information, including
medications, DME and Plan of Care into the Electronic Medical
Record (EMR) system Developing positive and effective relationships
with physicians, referral sources, insurance case managers, nursing
homes, patients, families, and coworkers Providing direct nursing
care to patients, in order to maintain the highest level of care
for the patient under the direction of the primary care physician
Participating in the admission nurse rotation on weekends, and
assists in covering SNV, as needed Communicating effectively and
professionally within the team environment, demonstrating
respectful and appropriate language and nonverbal behavior;
actively seeking to resolve customer and co-worker concerns in a
timely manner. Demonstrating the ability to recognize, understand,
