Job Summary:
The SNF Director of Clinical Operations (SNFDCO) mentors and
educates the providers working in the Post-Acute Care setting
assigned to his/her regional area on company process for providing
care in that setting. The individuals in this role educate
providers to ensure the delivery of quality patient care and ensure
compliance with regulatory and clinical policies and procedures.
The SNFDCO is responsible for ensuring quality reviews are
completed at a Regional level with the contracted Post-Acute
facility owners. This position also coordinates the regular
clinical education plan for the entire group of post-acute
providers to include initial training for new providers, ongoing
training for current providers identified as needing clinical
coaching, as well as assisting the Post-Acute Account Manager with
ensuring the Healogics Specialty Physicians (HSP) Regional Medical
Director and/or Collaborating Physician support Medical Decision
Making.
This position is assigned a territory that spans 8+ states where
there are existing Healogics Post-Acute facility contracts. Any
territory may include 30+ providers, whereas new states and
providers may be assigned with growth.
The relative territory this position will oversee is Iowa,
Missouri, Kansas, Oklahoma & Texas.
All Healogics employees must perform their job responsibilities
according to all Healogics policies, Hospital policies, as well as
to accrediting organizations, federal and state regulation, and to
the Centers for Medicare and Medicaid Services (CMS) guidelines, as
applicable.
Essential Functions/Responsibilities:
- For new Post-Acute Providers:
- Participates in the interview process for new providers as
requested
- Assists in coordination of initial training and mentoring
programs, including iHeal, rounding process, basic needs of partner
facilities, policy and procedure and coordinates documentation
training with Revenue Cycle Manager
- Ensures provider has necessary supplies for patient care in the
Post-Acute setting
- When possible, is on site when provider starts providing care
to first patients
- Conducts ongoing calls with the new provider to ensure
consistency in quality of care as well as to train and reinforce
regulatory and clinical policy and procedure
- Works with staff at contracted Post-Acute facility to ensure
the rounding process is efficient and conducive for quality
care
- For existing Post-Acute Providers:
- Consistently mentors, educates and develops providers in their
clinical role through best practices and compliance with
policy
- Assists them in developing and implementing action plans for
improvements in clinical operations and care delivery
- Coordinates with Post-Acute Account Manager and Vice President
of Post-Acute Care regarding the provider's clinical mentoring and
performance improvement plans
- Coordinates and assists with delivery of regular group
education including sourcing content and presenters
- Promotes the Healogics' Cultural Beliefs by coaching providers,
building relationships with the partner Post-Acute facility
leadership both locally and regionally, including the
Administrator, Director of Nursing, Medical Director and other
influential decision makers.
- Recommends, plans and implements needed changes as appropriate
in administrative and clinical policies, procedures, systems to
support Healogics Key Results
- Partners with Vice President of Post-Acute Care, Post-Acute
Account Managers and Revenue Cycle Manager to review clinical and
operational outcomes and trends to improve overall process and
opportunity for improvement
- Supports initiatives and projects as directed by Vice President
Post-Acute Care, and Healogics senior leadership
- Performs other duties as required
Required Education, Experience and Credentials:
- Bachelor of Science in Nursing (BSN) and five (5) or more
years' experience in clinical operations, with increasing levels of
responsibility preferred
- Master's degree in Nursing, Healthcare Administration or
Business Administration preferred
- Current RN license - to be maintained throughout the duration
of employment in this position
- Demonstrated competency in BLS (Basic Life Support)
- Supervisory/management experience, preferred
- Experience in Post-Acute Care, preferred
- Prior experience in coding and wound care, preferred
- All regular U.S. Healogics team members must be at least 18
years of age (may vary by state law).
Required Knowledge, Skills and Abilities:
- Demonstrated competency in BLS (Basic Support Life),
- Demonstrated knowledge of federal and state regulation and of
accrediting requirements for healthcare organizations
- Outstanding interpersonal, oral and written communication
skills, to include presentation skills on a one on one basis and in
a group setting
- Demonstrated relationship building and influential
skills

Power Point) and Internet navigation
- Working knowledge of Customer Relationship Management (CRM)
systems
- Ability to maintain a clean driving record
- Ability to travel 75% of the time to assigned regional
locations/centers

Salary for this position generally ranges between $89300-$100000.
This range is an estimate, based on potential employee
qualifications: education, experience, geography as well as
operational needs and other considerations permitted by law.
