$5,000 SIGN ON BONUS VNA of Kansas City Home Health and Hospice is seeking a full-time Registered Nurse to join our award-winning team and provide home health services in the Kansas City metro and surrounding areas.Our Registered Nurses are part of a legacy that spans over 133 years in this nonprofit organization that is the 6th oldest home health in the entire country. Through these many years, our nurses have been known for the highest level of skill that continues today in our medical and specialty teams. This position provides exceptional autonomy while being connected to a multi-disciplinary team with Managers who have all been Case Managers, and continue to support the team with all needs. Our nurse case managers have enjoyed being part of an organization that is dedicated to maintaining a separate "After Hours" Team, allowing case managers to work Monday through Friday.Benefits include:Medical, dental and vision plans start on the 1st day of the month following the month of employmentPTO starts to accumulate from first day of employmentIn addition to PTO, six paid holidays per calendar year403(b) with company matchAvailable shifts - Self-determined scheduleThe Work of the Home Health RN Float Case Manager Includes:Completes an initial, comprehensive and ongoing assessment of patient and family to determine needs.Case Management replacement (Float) if there is a need to cover an employee that is out sick or on PTO. We believe this allows consistency with patient satisfaction.Provides professional nursing care by utilizing all elements of nursing process and as defined in the state Nurse Practice Act.Develops a care plan that establishes goals, including the patient and the family in the planning process.Regularly re-evaluates patient and family/caregiver needs and revises the plan of care as necessary.Uses health assessment data to determine nursing diagnosis.Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician.Counsels the patient and family in meeting nursing and related needs.Provides health care instructions to the patient as appropriate per assessment and plan.Assists the patient with the activities of daily living and facilitates the patient's efforts toward self-sufficiency and optional comfort care.Acts as Case Manager when assigned by Clinical Manager and assumes responsibility to coordinate patient care for assigned caseload.Communicates with other members of the interdisciplinary team to facilitate optimal care of the patient/family.Applicants must have:A valid Missouri and Kansas RN License to Practice.Current CPR Sign On Bonus applies to New Hires OnlyEOE
